If you are a Texas Resident receiving medicaid benefits you may be eligible for two hearing aids at no cost to you
Here is what you have to do:

1. CONTACT US. We will need your current medicaid card and a photo ID
2. Come in for a FREE Hearing Test. Bring your current medicaid card and photo ID. If you have not used your benefit and you have a qualified hearing loss we will help you get hearing aids.
3. PRINT THIS FORM and take it to your doctor. Your Doctor will examine you to see if you can be fit with hearing aids.

Note: This form must be signed by a doctor before you can be Fit with Hearing Aids.
